Identity Verification Forms
What you need to complete and send us
If you want to verify individuals using Australian identity documents, please complete the following two forms:
Send the completed forms to support@2shakes.co.nz. Processing usually takes a few days. We will email you once you have been approved, after which you can perform biometric ID verification in 2Shakes.
Australian DVS Form
The Australian Document Verification Service (DVS) form enables a 1:1 digital match of the biographical information on identity documents against records held by the original issuing agency. When you use biometric ID verification in 2Shakes, the DVS checks whether the details on an Australian driver license or passport match the information held by the issuing agency.
Tips for completing the form
Complete the form electronically. All fields must be typed, except for your signature. You can use PDF software such as Adobe Acrobat to complete the form.
Complete the General Information section on page 2 with a summary of your business and an explanation of why you need access. For example:
“We are an accounting, bookkeeping and tax agent business and need access to meet ATO, TPB and AML obligations.”The DVS require you to have a Privacy Policy. If it is available as a link on your website then the form includes a field where you can enter the link. If it is not on your website you will need to send a copy of this to us.
If you don’t have a privacy policy yet then you will need to create one to apply. There are many online tools that can help create a privacy policy, for example here is a link to a policy generator: https://www.pulsify.tech/free-tools/privacy-policy
Check the form carefully, as applications may be declined if there are errors. We will also check the form before submitting it.
Sign the form before sending it to us.
The Australian DVS form includes a consent section containing the wording that your clients will agree to during the Biometric ID Verification process.
Centrix Subscriber Agreement form
You also need to complete the Centrix form so that 2Shakes biometric identity verification can include addition identity and address verification, political exposed person checks, sanctions watchlist checks and adverse media checks.
Tips for completing the form
Complete the form electronically. All fields must be typed, except for your signature.
Do not select any additional checkboxes. Identity Verification has already been preselected. Do not select any other services, as those options relate to New Zealand-based services.
Sign the form as the subscriber. The signature field is somewhat hidden, so make sure you sign in the correct location shown below:
Info Security Policy Help
You do not need to send your Information Security Policy with your forms. However, you may need to provide it if requested.

Risk Management Policy Help
You do not need to send your Risk Management Policy with your forms. However, you may need to provide it if requested.
If you do not already have a Risk Management Policy and are considering creating one, you may wish to consider:

How can you mitigate those risks?
How will staff know how to respond to these risks?
